When something is clearly the result of deeper more important underlying factors, practitioners are reluctant to accept them as "root causes." These include:
- Failure to follow a certain provision of a certain procedure
- A specific instance of inadequate training
- The failure to identify a specific hazard
- The failure to adhere to a specific requirement
- The absence of a specific requirement
